Please Bring the Following Information to Your Appointment:

• All radiology studies pertaining to your current medical problem. They must be taken within the last 12 months and must include either X-rays, MRI or a CT scan.
• Prior medical records, such as operative reports, EMG and nerve conduction results. Also include consultations from other physicians, particularly those from your referring physician.
• A complete list of all medications you are currently taking.
• Your insurance card, co-payment, and billing form. You may want to contact our billing company, MedNet, at (310)322-4278 if you have any questions regarding coverage by your insurance plan.
• If Worker’s Compensation is involved, then written pre-authorization is required by your employer’s insurance company. Please bring the written pre-authorization with you to streamline the paperwork and payment process.
• The names, phone numbers, and addresses of ALL physicians involved in your care. Please be sure to include primary care physicians.
• The completed New Patient Questionnaire (click link to display for printing.)
• The completed Pain Drawing Form (click link to display for printing.)
• The completed Functional Questionnaire (click link to display for printing.)
• The completed General Review Form (click link to display for printing.)

Emergency Symptoms

The vast majority of back problems improve on their own or with nonsurgical treatment. There are a few warning signs, however, that may indicate serious spinal problems. If you experience any of these symptoms, seek medical attention immediately. Click here to learn more.

Non-surgical Options

About 80% of back and neck pain symptoms can go away on their own with non-surgical treatment options. That’s the good news. Click here to learn more. The bad news is that if you are in the other 20%, you need to recognize which symptoms need a spine specialist fast. Symptoms like loss of control of bowel or bladder, or weakness and numbness into an arm or leg, are all emergency symptoms and you need to see a spine surgeon within 48 hours. Otherwise these symptoms can become permanent.
